Absence Form

Sickness & Emergency

 

If your child becomes ill in school we will try to contact the first named person on the school’s contact list.  If for any reason this person cannot be contacted we will try contacting other names on the child’s contact list. 

 

It is essential that you notify the school office of any change of telephone numbers for the above reason.  We would also ask you to make other contact names available to the school for our records.

 

If a child has a serious accident while at school Parents will be contacted immediately. If we are unable to get in touch with parents, the school will contact the emergency services for treatment. All accidents involving children are recorded in the School’s Accident Book and an Accident Report will be sent home to be signed by parents.

 

If a child has a minor accident in school then he/she will be attended by a member of staff trained in First Aid. An Incident Report Form will be sent home with the injured child. Parents are asked to read the form and add any additional comments. Parents are then requested to sign and return the form to school.

 

 

Medicines in school

The condition of children requiring special medical attention should be made known to the principal or the class teacher at the very earliest opportunity. 

 

Prescribed medicines should be left into school with clear written instructions for their administration to the child.  Parents are welcome to come to school to administer any necessary medicines or to inform staff of correct procedures for doing so.
